Community Referral Specialist

Coordinates patient care in a way that ensures patients receive timely care and is sensitive to the need for multiple visits. Schedules patient's post-hospitalization discharge follow-up appointments/tests/procedures within requested time frame.

Responsibilities/Job Description:

Receives and processes requests from referring community physicians and clinics. Coordinates the visit/test/procedures in the timeframe that the referring physician and the specialist feel is necessary. Schedules patient's post-hospitalization discharge follow-up appointments/tests/procedures within requested time frame. Coordinates all necessary appointments while focusing on excellent patient relations and ensuring compliance with Physician discharge orders. Maximizes the use of UMMC/UMP physicians as referral sources. Coordinates patient care in a way that ensures patients receive timely care and is sensitive to the need for multiple visits. Ability to stay focused and work independently to ensure timely and accurate work in addition to a high level of productivity.

 

Mon-Fri 8:30-5pm

Responsibilities

• Makes outbound call attempt to patient following the Referral Specialist standard work.
• Uses the system protocols to appropriately schedule the patient, contributing to the enablement of collecting expected payment.
• Uses the escalation workflow when clinic access is a barrier to a patient being seen within the recommended time frame
• Ensures patients obtain appointment with desired/appropriate specialty
• Manages hospital discharge orders, arranging appointments, tests and procedures as indicated
• Manages MyChart messages and online appointment requests.
• Uses the system protocols to appropriately schedule the patient.
• Uses the escalation workflow when clinic access is a barrier to a patient being seen within the recommended time frame

Required Qualifications

• 1 year experience in patient scheduling and registration in a healthcare setting
• 2 years years of customer service experience.

 

Qualifications:

$22.96- $32.42 Hourly
