The Community Paramedicine Specialist plays a key role in assisting patients to successfully complete their health care plans and achieve their health care goals by overcoming barriers they encounter in their homes and communities. Through interactions with patients in their home, the Community Paramedicine Specialist assesses individual health care needs, identifies barriers to success, and woks collaboratively with a variety of internal (health system) and external (community) resources to develop individualized plans. The Community Paramedicine Specialist is responsible for providing tailored education to improve health literacy and promote continuity of care with patients' health care team. The Community Paramedicine Specialist works to connect underserved populations with available resources and is particularly attentive to the social determinants of health (SODH) that may impact their clients. A primary goal is to reduce the dependency of patients on the emergency care system, including the 911 system, and to reduce overutilization of the emergency department (ED) and unplanned hospital admissions.
MINIMUM REQUIREMENTS
Education: Bachelor''s Degree preferred. In lieu of degree, extensive prehospital experience may be considered.
Experience: 3 years of experience in a relevant clinical environment or as a released Attendant-in-Charge with a Fire or EMS transport agency preferred.

The pay range for this role is $23.77 - $47.53 hourly. Individual compensation will be determined by the selected candidate''s qualifications, previous work experience, and/or education.
