General Summary:
The Community Outreach & Events Specialist provides support for the hospital's community outreach, community benefit and social determinants of health initiatives. This role is responsible for coordinating day-to-day outreach activities, supporting community health worker programs, coordinating mobile health unit operations, and supporting the implementation of Community Health Needs Assessment (CHNA) priorities.
Reporting to the Manager of Community Outreach, the specialist translates enterprise strategy into effective, community-based programs that improve access, address identified needs and strengthen the health system's presence within the communities it serves. The role works closely with internal teams, community partners, and frontline outreach staff to ensure programs are well-coordinated, compliant, and responsive to community needs.
Responsibilities:
Supports the planning and execution of community outreach initiatives, health education programs, screenings, and community-based services aligned with CHNA priorities and Community Benefit objectives.
Coordinates outreach efforts focused on addressing social determinants of health and reducing barriers to care for underserved and vulnerable populations.
Works collaboratively with clinical and operational teams to identify outreach opportunities that engage staff in meaningful, community-focused initiatives.
Provides support to Community Health Workers, ensuring alignment with program goals, referral workflows and community engagement strategies. Supports onboarding and training of Community Health Workers.
Coordinates and oversees mobile health unit deployment, including scheduling, staffing, community site coordination, and logistical planning.
Serves as the primary operational point of contact for mobile health outreach activities, ensuring alignment with community priorities and internal partners.
Tracks and documents community outreach and community benefit activities using CBISA or other designated systems. Educates and supports internal stakeholders on documentation requirements and best practices.
Assists in the preparation of Community Benefit reports, CHNA implementation updates, and impact summaries in collaboration with the manager and internal partners.
Monitors program performance, tracks key outcomes and supports reporting for leadership, regulators and community stakeholders. Prepares presentations, reports, and updates to communicate outreach activity, outcomes, and community impact.
Supports the development and maintenance of community partnerships with nonprofit organizations, schools, faith-based groups, and local agencies. Represents the organization at community meetings, outreach events, and coalitions as assigned. Coordinates outreach activities with community partners to ensure effective engagement and efficient use of resources.
Provides day-to-day guidance and support to assigned outreach staff and community health workers. Assists with scheduling, workload planning, and coordination of outreach teams and resources.
Co-leads various Bayhealth committees and subcommittees focused on health equity, social determinants of health, and community outreach. Mobilize team responsible for representing Bayhealth in the community. Works with internal and external partners to address health inequities as part of community relations and outreach efforts. Works with the community outreach committee for event strategy, marketing, communications, and staffing.
Ensures outreach activities are executed professionally, safely, and in accordance with organizational policies and procedures.
Collaborates with Marketing & Communications to support promotion and storytelling related to community outreach initiatives.
Performs other duties as assigned within the scope and responsibility of the role.
Required Education, Credential(s) and Experience:
Education: Associate's Degree in Public Health, Health Administration, Healthcare Management, Social Work, Community Health, Nonprofit Management, Communications, or a related field is required.

Experience:
Required: Two (2) years of experience in community outreach, community health programs, nonprofit or healthcare-based initiatives, or related work.
Preferred: Five (5) years of experience in community outreach, community health programs, nonprofit or healthcare-based initiatives, or related work.
Preferred Education, Credential(s) and Experience:
Education: Bachelor Degree Public Health Or a Bachelor's Degree in Health Administration (MHA/MBA), Social Work (MSW), Public Policy, or a related discipline.
