The Department of Family Medicine and Rural Health educates and mentors future health professionals, provides compassionate patient care, conducts meaningful research and serves communities throughout Florida. From the classroom and clinic to research and community engagement, our faculty, staff and learners share a commitment to advancing medicine, improving health and preparing the next generation of health care leaders.

Responsibilities

• Provide outreach and engage members of the unsheltered population.
• Deliver health education tailored to the needs of unsheltered individuals.
• Link individuals to healthcare, social services, and other resources.
• Communicate health information using applicable language and concepts.
• Maintain up-to-date knowledge of community resources and available services.
• Build and sustain relationships with community members, organizations, and partners.
• Track and document project activities and metrics; complete required reports.
• Participate in trainings on improving health outcomes, homelessness, chronic illness, substance use disorder and treatment, and maternal and child health.
• Collaborate with others, including other team members, in improving health outcomes.

Qualifications

• High school diploma/equivalent and three years of experience. (Post-high school education can substitute for experience.)
• Commitment to community engagement, quality healthcare and wellness.
• Sense of awareness, humility and responsiveness.
• Experience in community education and outreach.
• Knowledge of local community agencies and resources.
• Ability to work with a variety of individuals.
• Strong teamwork and collaboration skills.
• Ability to establish positive, supportive relationships with community members and colleagues.
• Strong interpersonal and communication skills.
• Ability to work nights and weekends, as needed, to assist with community events.
• Valid FL or GA driver's license, or the ability to obtain prior to hire.

Preferred Qualifications

• Certified Community Health Worker (CHW).
• Lived experience or strong connection to the Big Bend region.
• Experience working with unsheltered homeless populations.
• Knowledge of public health topics, including health outcomes, quality care, and barriers to care.
• Familiarity with behavioral health, substance use disorders, and chronic disease management.
• Experience with data collection and reporting for community health initiatives.

Anticipated Salary Range

$25 per hour, commensurate with experience and education.
