JOB SUMARY
A Home Health Community Liaison Assistant drives patient census growth by building relationships with physicians, hospitals, and assisted living facilities to facilitate referrals
DUTIES & RESPONSIBILITIES
- Build and maintain relationships with referral sources such as discharge planners, doctors, and nursing home administrators.
- Promote home health, hospice, or home care services to potential clients and medical professionals.
- Attend community events, health fairs, and networking functions to increase agency visibility.
- Coordinate with hospital staff, families, and internal teams to facilitate smooth patient transitions from hospital to home.
- Maintain records of sales activity and track data related to patient referrals to ensure company growth goals are met.

JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)
- Previous experience in home health, hospice, healthcare marketing, or community outreach is often preferred.
- Exceptional interpersonal and communication skills for interacting with healthcare professionals and patients.
- Ability to develop a rapport with local stakeholders and prospective clients.
- Reliable transportation and a valid driver's license for travel within the assigned territory.
- Some positions may require a bachelor's degree in marketing or a related field
$20.00 - $24.00 Per Hour
