TITLE: REGISTERED NURSE HOSPICE CASE MANAGERSUPERVISION RECEIVED: COMMUNITY HOSPICE NURSING SUPERVISORSUPERVISION GIVEN: HOME HEALTH AIDES, LPN'SGENERAL STATEMENT: Assumes responsibility for overall management of hospicepatients/families/caregivers. Along with the interdisciplinary group, assesses plans andformulates and individualized nursing care plan and comprehensive services to patients/families/caregivers. Participates in on call rotation every 10 weeks fielding calls for hospicepatients after hours.
