    Summary

    Community Health Workers are individuals who have a unique background and skill level from their experience in using services and supports to achieve their personal goals of community membership, independence, health wellness, and productivity. Under immediate supervision of the Clinical Supervisor, the CHW serves as an advocate to individuals we serve; provides information and health support. The CHW performs a wide range of tasks to assist individuals we serve in regaining control over their own lives and over their own recovery process.

    ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:

    • Teach and role model the value of every individual's recovery experience.
    • Screens individuals for SDOH that may contribute to disease and/or present as barriers to self-management
    • Coordinates and provides access to chronic disease management
    • Implements wellness and prevention initiative
    • Facilitates health education groups
    • Provides education health conditions and strategies to implement care plan goals, including both clinical and non-clinical needs
    • Accompanies individual to appropriate and supports groups
    • Coordinates and provides access to individual and family supports, including referral to community supports groups
    • Meets regularly with the care team to plan care and discuss cases and provide feedback as needed to promote treatment success
    • Assist individuals we serve in articulating personal goals for recovery.
    • Assist individuals we serve in determining the objectives they need to reach their recovery goals.
    • Attend staff meetings to provide information and training BHH as directed by supervisor
    • Assist individuals we serve in creating a Wellness Recovery Action Plan (WRAP) and/ or relapse prevention plan.
    • Utilize and teach problem solving techniques with individuals and groups.
    • Attend professional advancement opportunities to maintain skills.
    • Other duties as assigned.

    QUALIFICATIONS REQUIREMENTS

    EDUCATION AND/OR EXPERIENCE      

    High School Diploma or GED required.  Preference of a certified Community Health Worker and Peer Support Specialist Certification, the ability to obtain CHW certification within 6 months of employment is required.     LANGUAGE SKILLS

    Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.  Ability to write routine reports and correspondence.  Ability to speak effectively before groups of customers or employees of organization.

    MATHEMATICAL SKILLS

    Basic Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.  Has the ability to compute rate, ratio, and percent and to draw and interpret bar graphs.

    REASONING ABILITY

    Intermediate Skills: Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.  Ability to deal with problems involving several concrete variables in standardized situations.

    COMPUTER SKILLS                           

    To perform this job successfully, an individual should have knowledge of Word Processing software. Typing speed of 45 wpm is recommended.

    Retirement

     

     

    Nationwide 401(a)-

    If hired on or after 9/1/2019 the Authority shall contribute an amount equal to three percent (3%) of the employee’s base wage.

     

     

     

     

     

    Pension-

    For Employees hired prior to 09/01/2019, the pension formula will be a two percent (2%) multiplier and the Employer will continue to provide contributions as actuarially determined to insure the financial soundness of the fund.

     

     

     

    Vested in the Authority’s contributions once thirty-six (36) months of service are completed with the Authority.

     

     

     

     

     

    Vested in the Authority’s contributions once eight years of service are completed.

     

    The Authority shall contribute fifty cents ($0.50) to the 401(a) Plan for each dollar ($1.00) an Employee contributes to his or her 457B Plan up to the first four percent (4%) of the employee’s base wage.

    The Authority shall not contribute more than five percent (5%) of the employee’s base wage for its total contributions under this section.

    Tuition Reimbursement

    May be reimbursed up to $5,250 annually.  *Requires approval and funding availability limited spaces approved each year / contingent upon funding & Agency need.

    *Note if you do not need medical insurance, $233.00 per month (less Social Security & Medicare Taxes) will be deposited into an annuity account.  You will need to set up the annuity account through Nationwide Retirement.

    For time off, it is calculated upon the number of hours worked – to reach the maximum benefit, you must work 40 hours per week.
