This is an onsite position in which the Community Health Worker will support our Transitions of Care Team.
Position Summary:
The Community Health Worker/Patient Navigator (CHW) will work to outreach, identify, recruit, and support patients to engage in care and/or care management services; and will provide patient education and advocacy to assist patients with accessing services of FHCW and make and follow-up on referrals for external services as part of the population health team. The Community Health Worker will support the Care Management Team and the Population Health Team.

Education and Experience:
Minimum of two years relevant work or college-level internship experience required. Proven ability required to perform patient interviews, coordinate and collaborate with internal services and external community partners. Bilingual in English and another language appropriate to FHCW patient population required.
Community Health Worker Core Training required within first year of employment.
