Baptist Medical Center is seeking a Community Health Worker to join its care coordination team at its Downtown Jacksonville location. This is a full-time, day-shift opportunity focused on improving patient outcomes and strengthening community health.

Community Health Worker, Job Responsibilities:

• Serve as a bridge between healthcare providers, community service organizations, and patients to improve care coordination and access.
• Build trusted relationships within the community by understanding patients' cultural, social, economic, and language needs.
• Support patients within a primary care medical home model, ensuring continuity of care.
• Provide health education and guidance for managing chronic conditions such as Diabetes, Cardiovascular disease, and COPD
• Help reduce health disparities and improve overall quality of life for the populations served.

Experience Requirements:

1-2 HealthCare experience

Education Requirements:

High School Diploma
