Southern Jersey Family Medical Centers, Inc. is looking for candidates with talent, professionalism and compassion. We are currently seeking Community Health Workers to improve health outcomes in Atlantic County, Burlington County and Salem County.

Summary:

The Community Health Worker (CHW) is a member of the Outreach Team responsible for engaging community members and supporting access to preventive care, chronic disease management, health resources, and social services. This role provides community-based outreach, care navigation, and support through Mobile Medical Unit activities and other engagement strategies.

This position supports the organization’s mission as a Federally Qualified Health Center (FQHC) by improving access to care, addressing social determinants of health, and supporting underserved populations, in alignment with HRSA Health Center Program requirements.

Hours: This is a full-time position that requires the ability to work a flexible schedule. General daytime hours with some weekends.

Position Responsibilities/Duties:

  • Community Outreach & Engagement

    • Conduct outreach and patient engagement in rural and medically underserved communities served by SJFMC
    • Build and maintain relationships with community organizations, partners, and local stakeholders
    • Participate in community events, health fairs, and outreach activities to promote access to care

    Care Navigation & Patient Support

    • Assist patients in accessing primary care, specialty care, behavioral health, and community-based services
    • Identify and address barriers to care, including transportation, insurance, food insecurity, housing instability, and digital access
    • Provide appointment reminders, referral tracking, and follow-up outreach
    • Support patients in navigating telehealth services and accessing virtual care

    Health Education & Prevention

    • Promote preventive care, chronic disease self-management, and adherence to care plans
    • Provide culturally competent, patient-centered education to diverse populations
    • Support patient understanding of available services, including sliding fee discount program options, as applicable

    Mobile Medical Unit Support

    • Assist with mobile medical outreach events, including patient intake, flow, education, and navigation
    • Support coordination of screenings and follow-up services

    Documentation, Reporting & Compliance

    • Document patient encounters, referrals, and outcomes accurately and timely in the electronic health record (EHR)
    • Support data collection and reporting activities related to program performance and Uniform Data System (UDS) measures, as applicable
    • Participate in quality improvement initiatives and program evaluations
    • Ensure compliance with HIPAA, organizational policies, and all applicable federal, state, and HRSA requirements

    Operational Support

    • Assist in coordinating outreach events and program activities
    • Maintain awareness of community resources and services
    • Participate in team meetings and training sessions
    • Perform other duties as assigned

Required Experience:
  • POSITION REQUIREMENTS

  • High school diploma or equivalent required

  • Experience in community outreach, health education, or social services preferred

  • Familiarity with electronic health records (EPIC preferred)

  • Bilingual (Spanish) preferred

  • Strong verbal and written communication skills

  • Ability to engage and build trust with diverse populations

  • Ability to follow established processes and work under supervision

  • Flexibility to work evenings and community-based schedules as needed

  • Valid driver’s license and reliable transportation
