Summary:
The Community Health Worker (CHW) is a member of the Outreach Team responsible for engaging community members and supporting access to preventive care, chronic disease management, health resources, and social services. This role provides community-based outreach, care navigation, and support through Mobile Medical Unit activities and other engagement strategies.
This position supports the organization's mission as a Federally Qualified Health Center (FQHC) by improving access to care, addressing social determinants of health, and supporting underserved populations, in alignment with HRSA Health Center Program requirements.
Hours: This is a full-time position that requires the ability to work a flexible schedule. General daytime hours with some weekends.

Care Navigation & Patient Support
Health Education & Prevention
Mobile Medical Unit Support
Documentation, Reporting & Compliance
Operational Support
Assist in coordinating outreach events and program activities
Maintain awareness of community resources and services
Participate in team meetings and training sessions
Perform other duties as assigned
POSITION REQUIREMENTS
High school diploma or equivalent required
Experience in community outreach, health education, or social services preferred
Familiarity with electronic health records (EPIC preferred)
Bilingual (Spanish) preferred
Strong verbal and written communication skills
Ability to engage and build trust with diverse populations
Ability to follow established processes and work under supervision
Flexibility to work evenings and community-based schedules as needed
Valid driver's license and reliable transportation
