JOB SPECIFICS

Washington County is currently hiring for a Community Health Worker. The Community Health Worker assists individuals in accessing and navigating health and social services, with a focus on all aspects of health and wellness for an individual, family and the community. They will serve as a liaison between county services and the individual/community to simplify the process to access services, improve the quality and cultural competence of service delivery while building community capacity. Bicultural and/or bilingual candidates with close ties to cultural communities are encouraged to apply. Current need is for candidates with a close connection to the Spanish Speaking Community.

DESIRABLE EXPERIENCE:

• Experience as a Community Health Worker
• Experience (paid, unpaid or volunteer) with community engagement, providing culturally appropriate resources information, participating in community projects, and/or coordination of care to individuals and groups within diverse communities. This includes face-to-face, phone or group interactions, and may include traveling to an individuals home, community locations, various agencies, and other outreach destinations
• Experience (paid, unpaid or volunteer) building relationships in a community-based setting with local communities and/or community-based organizations (such as schools, community, faith-based organizations, and health care facilities)
• Experience providing services to individuals from a variety of cultural and ethnic backgrounds

SELECTION TECHNIQUE: Your training and experience will be rated based on the required application material submitted and the top-scoring candidates will be forwarded to the hiring department for further consideration.

HOURS / DAYS: 8:00 am - 4:30 pm / Monday - Friday plus overtime hours as needed. Work schedules (hours and days) are subject to change based on department needs.

ANTICIPATED PAY RATE: $28.62 - $39.00 Hourly

JOB SUMMARY

EMPLOYMENT CRITERIA

MINIMUM QUALIFICATIONS:

• High school diploma or GED with at least 3 years of experience (combination of paid and unpaid) in human services or related field.
• or completion of at least two years of post-secondary education with at least two years of experience (combination of paid and unpaid) in a human services or related field.
• or completion of Community Health Worker certificate through an approved college or technical school and two years of experience (combination of paid and unpaid) in a human services or related field.
• Must have and maintain a valid class D driver's license.

DUTIES & RESPONSIBILITIES

1. Research, assess and identify county and community resources, in order to provide information, outreach, and coordination of care to individuals and professionals within diverse communities, while following protocols and policies for assigned duties. This includes face-to-face, phone or group interactions, and may include traveling to an individual's home, community locations, various agencies, and other outreach destinations.

2. Build new and maintain current - relationships and communications with community members, local organizations, schools, community, faith-based organizations, and health care facilities. Collaborate with these partners to address the identified needs of the community.

3. Collaborate with county partners to address identified needs of the community.

4. Educate, translate and promote wellness by providing culturally appropriate health information to individuals and groups such as health literacy, physical activity, nutrition, tobacco, vaccinations, etc.

5. Educate, translate and promote information related to the social determinants of health such as employment, housing, transportation, financial assistance, etc.

6. Determine data gaps and assist in gathering data to identify community strengths and assets as well as supporting needed services.

7. Advocate needs of the community and individuals to county staff to assist in the evaluation of the individual's needs or input for community health projects.

8. Provide cultural consultation and assist with development of changes in departmental policy/procedures that better meet the needs of the community.

9. Maintain accurate, concise and timely documentation of completed work.

10. Lead community engagement meetings and events and represent Washington County in person or virtually.

11. Performs public health emergency response functions as trained and assigned within the department's emergency response plan.

WORK ENVIRONMENT:

Semi-sedentary involving office and outside field work. Lifting requirement may include lifting up to 50 pounds. Exposures may include but not be limited to disagreeable odors, communicable diseases, bodily fluids and behavior challenges. Use of general office equipment to include telephone, cell phone, personal computer, laptop computer, electronic recorder, video recorder, mobile microphone, calculator, etc. Additional physical/mental requirements that occur may not be listed above but are inherent in performing the positions essential functions.
