Rural Health Transformation Program - College of Medicine
Florida State University's College of Medicine has been awarded more than $19 million through the Rural Health Transformation Program to strengthen healthcare access and outcomes in rural Florida. The funding will support six new projects focused on critical needs including behavioral health, preventive care, food insecurity, telehealth, at-home care and healthcare workforce development. Through partnerships with communities and healthcare organizations across Northwest and Southwest Florida, FSU is working to bring innovative, practical solutions directly to rural communities and help close longstanding gaps in care.

This role will be the primary accountability bridge across five Rural Health Transformation Program initiatives that form a network to expand access to preventive and primary care services for rural populations. The initiatives are (1) mobile health, (2) on-site pharmacy/retail clinic services, (3) community paramedicine, (4) remote patient telemonitoring and (5) health information exchange.

This position will be responsible for traveling throughout Walton County and will support Bundle 1 - Preventative Care & Care At Home Northwest region, which is defined as Bay, Calhoun, Escambia, Franklin, Gadsden, Gulf, Holmes, Jackson, Jefferson, Leon, Liberty, Okaloosa, Santa Rosa, Wakulla, Walton, Washington on the Rural Health Transformation Program Map.
