The Community Health Worker - On Call is responsible for developing and delivering activities designed to promote individual, community and population health with a focus on social determinants of health. In this exciting and dynamic role, your key responsibilities will include referral outreach and care coordination, health education, community engagement and health promotion across both community and clinical settings, such as medical, dental, pharmacy, behavioral health.
This on call position is responsible for Community Health Worker (CHW) coverage for all 7 primary care clinics within the Portland Metro area. The individual must have flexibility with their site assignments, as the employee may be asked to change clinic location with short notice.

Serving the Public, Even During Disasters:
Everyday, Multnomah County staff work together to serve as a safety net for our communities. During a disaster, this safety net becomes even more critical. All County employees have a role in serving the public during inclement weather, natural disaster, or other types of community emergency response. During these emergency responses, while typically there begins with a call for volunteers, county employees may ultimately be reassigned from their current position to a role in the emergency response in order to support the critical needs presented by our communities.
