JOB SUMMARY:     Support Crisis Workers and Program Coordinators in managing daily tasks and duties of the program.

ESSENTIAL FUNCTIONS:      

  • Work alongside Program Coordinator to ensure timely completion of CSOC/Performcare tasks 
  • Act as a project manager for special projects, which may include planning and coordinating presentations, disseminating information and organizing events
  • Act as liaison with other departments and outside agencies
  • Assist and support CIS with the management of daily tasks and duties
  • Assist with finding providers and locating services per clinical recommendations
  • Assist with making referrals and linkages for services
  • Answer the phone, greets customers/visitors as needed
  • Accompany staff on dispatches in the community
  • Assist staff with BIO/billing/contacting families and other tasks as required
  • Record and document meetings as  needed
  • Design and type general correspondence, memos, charts, tables, graphs, etc.
  • Organize and prioritize large volumes of information and calls
  • Development and completion of program PDSA(s)
  • Chart Reviews of closed charts and reporting
  • Complete client intakes for each individual client
  • Maintain Intake Logs and Completion of Crisis Tracking in CYBER
  • Complete and submit Logic Model for the Program
  • Assist in gathering data in completion of Annex A
  • Facilitate & Coordinate Client stories with Supervisor and Staff
  • Oversee and ensure compliance with Client/Family Satisfaction Surveys
  • Run reports in Anaszi/Cerner & CYBER
  • Billing Clean Up and ensure timely addressing of issues
  • Assist with obtaining Program Consent Forms
  • Coordinate and Facilitate Flex Fund Purchases for Families as Needed
  • Verify Medicaid /3560 at beginning of each Month
  • Assist with completion of Court Petitions (FCIU) as needed
  • Assist with completion of FCIU reports (monthly)
  • Serve as back up for completion of PE applications & 3560
  • Provide backup coverage for Administrative Assistants
  • Coordinate & facilitate staff group activities and events
  • Schedulesand organize complex activities such as meetings, travel, and departmental activities
  • Report directly to and be supervised by the Program Coordinator and/or Program Supervisor. 
  • Seek and use supervision in a professional fashion
  • Carry out job responsibilities in keeping with contractual requirements.
  • Learn and use the CYBER Software programs that are used by the State system.   
  • Comply with all Council on Accreditation Standards of Catholic Charities.
  • Attend staff meetings; attend state and agency trainings; work cooperatively with staff; participate with all team assignments established by supervisor; serve on professional committees as required by administration.
  • Provide additional services as assigned by supervisor.
