We are seeking a compassionate and motivated Community Health Worker to help individuals overcome barriers to care and improve their overall health outcomes.Job SummaryThe Community Health Worker will serve as a link between community members, healthcare providers, and social service organizations. The role will focus on health education, community outreach, care coordination, resource navigation, and helping individuals access appropriate healthcare and support services.Key ResponsibilitiesConduct community outreach and build relationships with individuals, families, and community organizations.Educate community members about preventive care, chronic disease management, nutrition, wellness, and healthy lifestyle practices.Connect individuals with healthcare providers, insurance programs, transportation, housing, food assistance, and other community resources.Assist clients with scheduling medical appointments and accessing healthcare services.Identify barriers to healthcare and help individuals navigate available resources.Conduct basic health screenings and assessments within the scope of the role.Provide follow-up support to ensure clients receive recommended services.Support care coordination between patients, healthcare providers, and community organizations.Maintain accurate documentation of client interactions, referrals, outreach activities, and outcomes.Participate in community health events, educational programs, and wellness initiatives.Provide culturally appropriate health education and support.Assist public health and clinical teams with health promotion and disease-prevention programs.Maintain client confidentiality and comply with applicable privacy requirements.QualificationsHigh school diploma or GED required.Associate or bachelor's degree in Public Health, Community Health, Social Work, Health Sciences, or a related field preferred.Community Health Worker certification preferred or required where applicable. 1 2 years of experience in community health, healthcare, social services, patient navigation, or community outreach preferred.Knowledge of community resources and social determinants of health.Strong interpersonal and communication skills.Ability to communicate effectively with individuals from diverse backgrounds.Strong organizational and documentation skills.Ability to work independently and as part of a multidisciplinary healthcare team.Preferred SkillsExperience with community outreach and health education.Knowledge of preventive health and chronic disease management programs.Experience with patient navigation and care coordination.Familiarity with Medicaid, Medicare, and community assistance programs.Knowledge of social determinants of health (SDOH).Experience with electronic health records (EHR) or case-management systems.Bilingual or multilingual communication skills are a plus.Basic knowledge of HIPAA and patient confidentiality requirements.Working ConditionsThis position may involve a combination of office work, healthcare facility visits, community outreach, home visits, and participation in community events. Local travel may be required.
