Responsibilities:

Job Summary

Perform direct outreach to target population as listed on program grant and/or as directed by supervisor.

 

Essential Job Functions

• Provide basic information on health topics as directed by Supervisor
• Conduct comprehensive outreach (and/or in-reach) in local area, and refer individuals to program grant activities
and/or clinic
• Complete appropriate paperwork
• Conduct basic assessments/screenings of target population
• Complete data collection forms
• Participate in health fairs and other community events
• Track incentive cards
• Demonstrate familiarity with VCC programs and services
• Meet minimal requirements of program objectives
• Database entry
• Attend meetings, as needed
• Enhance professional growth and development through participation in educational programs, current literature
review, in-service meetings and workshops
• Support the vision, mission and goals, and demonstrate a commitment to the values, of the organization
• Perform other duties as directed

Qualifications:

Minimum

• High school graduate or equivalent
• Minimum of two years’ experience
• Bilingual English/Spanish
• Valid California driver’s license and vehicle insurance; reliable transportation, to travel for meetings and/or to gather
  information for reporting purposes; minimum two years’ driving experience; no more than 2 violations or a single
  ticket/accident value at more than 1 point on driving record

Required Skills/Knowledge/Abilities

• Demonstrated ability to communicate effectively orally with target population and VCC staff
• Effective verbal and written skills necessary for communication with patients/clients, clinicians and other staff
• Ability to interface with all levels of personnel in a professional manner, including people of all social, cultural and
  ethnic backgrounds and within the constraints of government funded programs
• Ability to work independently, take initiative, and be proactive
• Ability to track and manage time effectively
• Ability to handle confidential materials and information in a professional manner
• Ability and willingness to provide outreach throughout targeted areas including on the street, door-to-door, in retail
  outlets, businesses and other venues as needed
• Ability and willingness to meet the organization’s attendance and dress code policies

Pay Range

• $22.00 - $23.00 Depending On Experience
