Position Summary

The purpose of this position is to provide resources for Maternal-Child Health (MCH) issues in the City of York. This will be accomplished by care and education to target populations, as well as actively be involved with program development to meet present and future health needs of community involving Maternal-Child Health. Coordinating and collaborating with community partners to address Maternal Child Health concerns in the City. Meeting new/expectant mothers both pre-natal and post-partum to identify resource needs and health issues. Educate new/expectant mothers on state priorities around Safe Sleep, Breastfeeding and birth spacing. Developing, reviewing, and maintaining records.

Essential Functions

Serve as a resource to expectant mothers and new mothers pertaining to MCH issues. Conduct /guide home visits with women to educate on MCH issues, such as Safe Sleep practices, breastfeeding and birth spacing. Conduct depression screening tool for all women and refer to care as needed. Ensure women are linked to community services as needed. Keep, maintain and report on activities and outcome.

Work with Bureau of Health Leadership to asses and refine present program involving MCH, as well as, develop new program(s) and approaches to meet unmet or new health needs in community. Participate in meetings with local, state and regional MCH partners. Identify gaps and/or opportunities for collaboration and improvement of process. Recommend and/r implement strategies to improve MCH processes and birth outcomes. Ensure coordinated messaging and communications for MCH providers. Incorporate evidence-based approaches and best practices for MCH interventions to meet state and federal priorities.

Participate in cross-coverage of other clinical programs to include immunizations, tuberculosis, or other new areas as directed by leadership. Participate in Health Bureau training and exercises. Assist in clinical activities as needed and appropriate. Other duties as assigned.

Required Knowledge, Skills and Abilities

Associate or Bachelors degree in Nursing is required.

Commonwealth of Pennsylvania Valid Registered Nurse License required

Experience Required: Over three (3) years of related work experience

Requires knowledge of Maternal-Child Health issues, such as Safe Sleep practices, breastfeeding and birth spacing.

Minimum Acceptable Training and Experience

Valid Class C drivers license issued by the Commonwealth of Pennsylvania

Registered Nurse licensed by the Commonwealth of Pennsylvania.

All persons who shall become employed by the City shall be or become a resident of York County, or any County contiguous to York County (Adams, Cumberland, Dauphin, Lancaster, Hartford, Baltimore, and Carrol Counties), within six (6) months from the date the employee achieves permanent employment status.
