Under general direction of the Community Health Director, the Manager supervises the daily activities of Community Health Workers a in providing education, care coordination and access to resources for program clients to improve their health and well-being. Serves as the liaison to internal and external health projects. Works primarily in the community setting, either in client''s homes, health centers, or via telehealth when patient contact isn''t safe. Job Announcement Flyer: VA1372026 Community Health Manager.pdf [pdf - 432.42 KB] Duties: 1. Provide direct supervision to the program Community Health Workers. 2. Supervises community health workers, to include work allocation, training, and problem resolution; evaluate performance; motivate employees to achieve peak productivity and performance. 3. Assists in the development of training and continuing education to maintain certifications for Community Health staff. 4. Provide development and routine review of Standard Operating Procedures related to health services for the community health workers and staff. 5. Train new Community Health Workers and provide ongoing staff development plans and opportunities. 6. Review staff program reports for clarity, content, and spelling. 7. Provide case file reviews for completeness, reviewing plan of care, and documentation of case notes. 8. Assist with more complex cases, as needed, including assisting clients in their homes, community, or clinic setting, and documenting client encounters. 9. Assist CHW to build rapport and maintain positive working relationships with the clients, providers, agency representatives, supervisors and office staff. 10. Work with directors under HHSD to ensure care coordination and confidentiality for shared clients. 11. Collaborate in partnership with health professionals at Haak''u Health Center and other key health centers; specifically with Public Health Nurses, and Emergency Medical Service providers, when necessary. 12. Participate in ongoing continuing education programs to stay proficient in best practices, especially related to underlying health factors that contribute to elevated risk of post-pandemic chronic health conditions. 13. Contribute to programmatic support activities, including reporting, quality improvement initiatives, outreach, and creation of programmatic materials. 14. Assist in developing new health and wellness programs and revising current programs as indicated by program evaluation and demographic changes. 15. Coordinate workshops, training sessions, and community events to educate the community in various health related conditions (Face-face and virtual, depending on the public health status of the Pueblo). 16. Perform other duties as assigned.
