DEPARTMENT: Community and Employment Support Services
POSITION: Community Engagement Coordinator - CPS
STATUS: Full-Time; 40 hours / week
SCHEDULE: Flexible to meet the needs of the program (primarily daylight)
LOCATION: Robinson Township

 

Citizen Care Inc., a subsidiary of Partners For Quality Inc., provides residential, day program, habilitation, supported employment opportunities and behavioral support services to adults with intellectual and developmental disabilities. We are a non-profit organization, accredited by the Council on Quality & Leadership. Citizen Care has locations throughout the West Hills and South Hills of Allegheny County.

 

Responsibilities:
    • Assure that each individual’s personal outcomes, hopes and dreams are being identified and provide supports so that staff may assist people in realizing their goals
    • Demonstrate a working knowledge of people’s personal outcomes
    • Monitor people’s progress toward achievement of personal outcomes
    • Educate people about their rights and encourage people to exercise their rights
    • Demonstrate a positive working relationship with the Managers of Community and Employment Services
    • Initiate / coordinate meetings and the flow of communication to ensure continuity of services for people
    • Participate in the interdisciplinary design, development, implementation, and monitoring of the individualized plan for service and supports for people receiving services including:

     

      • All ISP duties of the Program Specialist position as outlined in the 2380 Regulations
      • Supervision, scheduling and administration of assessments
      • Three-Month Reviews
      • Design and implementation of Things I Want to Learn, Things to Know and Do and Therapeutic Support Plans
    • Coordinate implementation of interdisciplinary team recommendations
    • Writing of Minutes, Progress Notes and Incident Reports
    • Investigating and coordinating community bridging and linking activities
    • Support site Quality Improvement Plans
    • Assist people to participate in paid work opportunities
    • Maintain current and accurate information in the records of people receiving services

     

    • Assist in training staff in the completion of responsibilities that support the attainment of personal outcomes for people as demonstrated by but not limited to assuring that staff assist people to:        
    • Achieve / realize Personal Goals
    • Meet basic personal care needs
    • Meet health and dietary needs
    • Experience community quality of life
    • Have safety and security
    • Maintain accurate paperwork and record keeping
    • Work together with other staff to promote a safe, healthy and productive environment
    • Interact positively, at all times, reflecting dignity and respect for the person
    • Provide education and support for people to exercise rights

     

    • Provide results-oriented reports to the Manager of the Senior Citizen Center (Walden) by reflecting coordinated service delivery and day-to-day operations at the facility by:
    • Coordinating annual Plans for Services and Supports, Quarterly Reviews and Monthly Reviews in conjunction with the Director of Facility Operations
            • Exercising initiative and educated decision making
            • Communicating significant information (including transportation changes, behavioral concerns and medical issues) to all involved parties
            • Performing duties of Medication Administration Practicum Observer as certified by the Pennsylvania Department of Human Services, as required
            • Assuming management responsibilities for the operation of the facility in the absence of the Manager
            • Completing other related duties as specified
Qualifications:
  1. Associate’s degree or 60 credit hours from an accredited college or university and four (4) years of work experience working directly with persons with disabilities; OR

 

Bachelor’s degree from an accredited college or university and two (2) years of work experience working directly with persons with disabilities; OR

 

Master’s degree or above from an accredited college or university and one (1) year of work experience working directly with persons with disabilities;

 

 

2. One (1) year management/supervisory experience preferred

  1. Experience in designing and implementing services and supports plans
  1. Excellent communication and writing skills
  2. Excellent references, both work and personal
  3. Valid PA, OH, or West Virginia driver's license
  4. Act 34 Clearance and FBI Clearance, as applicable
Minimum Compensation: USD $50,000.00/Yr. Maximum Compensation: USD $59,000.00/Yr.

Exceptional Adventures has more than 40 years of experience providing vacation opportunities, social events and monthly dances for individuals with intellectual and developmental disabilities. Our recreational and travel programs are designed to support guests in a wide variety of activities. Organized trips include motor coach, flight, train and cruise packages which are offered year-round.

Milestone Centers supports individuals with behavioral health challenges and intellectual & developmental disabilities in communities throughout Western Pennsylvania. Our Agency oversees 20 residential settings, two outpatient offices, three adult training facilities, a prevocational enterprise program, a consumer drop-in center, habilitation services, and two Health Care Quality Units in Butler and Warren, which train professionals in 18 counties. Milestone Centers has earned certification from the Sanctuary Institute for providing trauma-informed care.

The Partners For Quality Foundation conducts fundraising for all organizational services and supports.  Partners For Quality Inc. receives operating funds through various sources, private gifts, grants and special events helping the agency go “above and beyond” basic needs. The Foundation’s goal is to build and strengthen PFQ’s support resources to supplement government dollars.
