    About the Role

    A Medicare and JCAHO-certified home health agency with a strong presence across the Midwest is seeking an experienced Community Care Program Manager to oversee clinical operations and care coordination at the site level. This is a hands-on leadership role for someone who thrives in a fast-paced, patient-first environment and can manage both people and program outcomes.

    Core Responsibilities
    • Oversee day-to-day operations of community care programs including scheduling, compliance, and care coordination
    • Supervise clinical and field staff — RNs, CNAs, and home health aides
    • Ensure compliance with Medicare, JCAHO, and state regulatory standards
    • Manage patient intake, care plans, and interdisciplinary team communication
    • Monitor program metrics, identify gaps, and drive continuous quality improvement
    • Coordinate with referral sources, discharge planners, and community partners to build census
    • Assist in recruiting, onboarding, and performance management of care staff
    Required Qualifications
    • 3+ years of experience in home health, community care, or a related healthcare operations role
    • Active RN license in Illinois or demonstrated healthcare management background
    • Working knowledge of Medicare/Medicaid compliance and JCAHO standards
    • Strong leadership and communication skills — experience supervising clinical or field teams
    • Proficiency with EMR/scheduling systems
    • BLS/CPR certification
    Benefits & Pay
    • Salary: $65,000 – $85,000 depending on experience
