Position Summary

As a Licensed Community-Based Therapist, you will provide intensive, evidence-based behavioral health treatment to children, adolescents, and families experiencing Serious Emotional Disturbance (SED) and complex behavioral health needs. Working as a member of the Youth ACT multidisciplinary team, you'll deliver services in homes, schools, and community settings using trauma-informed, family-driven, and youth-guided approaches that promote recovery, resilience, and long-term stability.

This position is ideal for licensed mental health professionals who enjoy working independently while collaborating closely with a dedicated clinical team to improve outcomes for youth and families.


What You'll Do

  • Conduct comprehensive behavioral health assessments and develop individualized treatment plans.
  • Provide evidence-based individual, family, and group therapy in home, school, and community settings.
  • Deliver crisis intervention, risk assessments, and safety planning to support youth and families during behavioral health emergencies.
  • Coordinate care with psychiatrists, nurses, schools, primary care providers, child-serving agencies, and other community partners.
  • Help youth build emotional regulation, coping, communication, and independent living skills.
  • Promote family engagement and strengthen support systems that encourage long-term success.
  • Participate in multidisciplinary team meetings, treatment planning, and shared caseload management.
  • Maintain accurate and timely clinical documentation in compliance with OMH, Medicaid, HIPAA, and agency standards.
  • Participate in required trainings, supervision, quality improvement initiatives, and rotating after-hours on-call coverage.
  • Support Liberty Resources' mission of providing high-quality, culturally responsive, trauma-informed behavioral health services.

What We're Looking For

Required Qualifications

  • Master's degree in Social Work, Mental Health Counseling, Marriage and Family Therapy, Psychology, or another behavioral health field.
  • Current New York State license as one of the following:
    • LCSW
    • LMSW
    • LMHC
    • LMFT
    • Licensed Psychologist
  • Experience providing behavioral health services to children, adolescents, and families.
  • Strong clinical assessment, treatment planning, crisis intervention, and care coordination skills.
  • Excellent communication, organizational, and documentation abilities.
  • Valid New York State Driver's License and reliable transportation.

Preferred Qualifications

  • Experience working in Youth ACT, ACT, Home-Based Therapy, Community Mental Health, Mobile Crisis, or intensive community-based behavioral health programs.
  • Experience supporting youth with Serious Emotional Disturbance (SED), trauma, co-occurring disorders, and complex behavioral health needs.
  • Knowledge of OMH regulations, Medicaid documentation, electronic health records, and evidence-based behavioral health practices.
  • Current CANS-NY Certification or willingness to obtain certification.
  • Proficiency with Microsoft Office 365, telehealth platforms, and electronic health record systems.
