Overview

At the Larimer County Department of Human Services (DHS), we strive to be responsive and provide timely resources in partnership with our community so that individuals and families are healthy, supported, and safe. We are seeking a highly motivated, innovative and enthusiastic staff member to join our Children, Youth, and Family Division as a Collaborative Management Program (CMP) Coordinator.

The CMP Coordinator will be responsible for facilitating and coordinating the work of the Larimer County Collaborative Management Program, the Executive Committee, and subcommittees, as well as ensuring that Larimer County meets the requirements for House Bill 1451. Although this position will remain housed within Larimer County DHS, it is accountable to Larimer County Interagency Oversight Group (LCIOG), the oversight group for Larimer Countys CMP program. The mission of LCIOG is to promote, maintain and restore community health, safety and well-being by partnering with families, victims and agencies to provide an integrated and efficient continuum of local services for children/youth, birth to 21. The CMP Coordinator will drive the work of the LCIOG, resulting in improved outcomes for children, youth and families in our community.

The typical schedule is Monday - Friday, 8:00 AM - 5:00 PM. This position is a hybrid position and will require at least one day/week in the office. You will be required to have adequate internet access and space for remote work. Larimer County provides a one-time remote work stipend to assist with equipment and office supply needs for remote workspaces. Three professional references with valid contact information are required for submission as part of this application.

Responsibilities

What youll be doing:

  • Coordinating and facilitating the work of the Collaborative Management Program (CMP), Executive Committee, and subcommittees to advance shared goals and strategic priorities.
  • Serving as the Countys subject matter expert on House Bill 1451 (HB1451), ensuring compliance with statutory requirements and representing Larimer County at the state level.
  • Building partnerships among elected officials, agency leaders, community organizations, and individuals with lived experience to support collaborative decision-making.
  • Managing program data, analyzing trends, and preparing reports, including the CMP Annual Report and other required state documentation.
  • Administering funding processes, monitoring compliance with HB1451 requirements, managing program budgets, and supporting grant activities.
  • Supporting the day-to-day operations of the Larimer County Interagency Oversight Group (LCIOG), including meeting coordination, board administration, member onboarding, and records management.

  • Three (3) years experience in a directly related field or in the performance of similar duties.
  • Possess reliable transportation to be able to work in any County facility as needed and required.

Supplemental Information

The position will be located at one of the following three locations: 1501 Blue Spruce Drive, Fort Collins, 2555 Midpoint Drive, Fort Collins, or 200 Peridot Avenue, Loveland.

All new employees and rehires will be required to submit fingerprints for a CBI criminal background check (c.r.s. 26-6-104).

All staff working in the After Hours Team will be required to submit information for a Colorado Adult Protective Services data system check (c.r.s. 26-3-1-111).
