    Job Duties:
    The Nursing Assistant works under the direct supervision of the Nurse Manager (RN/LPN/DDP) and is responsible for ensuring that the utmost in competent care and safety is consistently delivered to all clients.

    Required Qualifications:
    • High School Diploma or Equivalent.
    • Nursing Assistant Certificate.
    • First Aid/CPR (American Heart Association or Red Cross).
    • GACCP test and check off.
    • DCH/NCIC/DBHDD clearance.
    • Experience in a Home Health care facility is preferred, but not required.
    • Capable of lifting 50 pounds without assistance.
    • Maintains a current driver’s license if doing transportation.
    • Must have a positive attitude, flexibility, and ability to work with people of diverse backgrounds.

    Duties and Responsibilities:
    • Supports individuals in attaining personal goals and desires.
    • Ensures the safety and security of residents.
    • Turns and repositions bedridden individuals, alone or with assistance, to prevent bedsores.
    • Completes all progress notes and documentation before the change of shift.
    • Assist individuals with ADLs (supervising, feeding, bathing, and dressing).
    • Take vital signs (temperature, blood pressure, pulse, and respiration) as directed by the physician or nurse.
    • Collect samples and monitor catheters/feeding tubes as directed by the physician or nurse.
    • Provide wound care as directed by the physician or nurse.
    • Routine hygiene care throughout the day.
    • Supervise in addressing their ISP/Plan of Care goals and document progress.
    • Support all opportunities for the growth, learning, and development of the resident.
    • Complete daily progress notes highlighting person-centered events.
    • Complete understanding of the agency’s policies and procedures.
    • Attend all staff meetings.
    • Maintain individuals’ records on site in accordance with policy and procedures.
    • Attend mandated trainings as required.
    • Assumes full responsibility when at work for food preparation, community activities, and assisting individuals with other needs.
    • Supports individuals by planning nutritious meals and supporting those with special diets.
    • Cooperates with family members regarding reasonable requests related to the needs of individuals.
    • Supervises the self - self-administration of prescribed medication and maintains a record of medication by documentation on MARs according to established policies.
    • Maintains medical chart for each individual, consistent with the physician’s orders, and reports any medical emergencies according to the Medical Emergency Plan.
    • Administer medications to those individuals unable to self-administer (must be proxy trained).
    • Assumes responsibility for cash disbursements given by the supervisor and keeps receipts showing the actual amount spent, if applicable.
    • Supports individuals with learning and maintaining personal and household skills.
    • Provides physical supports to residents, including, but not limited to, lifting and moving adaptive equipment.
    • Receives visitors to the facility; maintains family family-oriented nature of the facility by being hospitable to family members and other natural supports by making arrangements as needed for family.
