Job Description

We are seeking an experienced and compassionate CNA / QMAP to join the team at Hillcrest Care Center & The Towers in Wray, Colorado. The ideal candidate will provide quality patient care and assist residents with their daily living needs while also performing medication administration duties in accordance with QMAP requirements and facility policies.

Responsibilities

• Provide direct patient and resident care in a long-term care setting.
• Assist residents with activities of daily living, including bathing, dressing, grooming, mobility, and toileting.
• Monitor and report changes in residents' physical or emotional condition to the appropriate healthcare team members.
• Administer medications as permitted under QMAP certification and applicable facility policies.
• Accurately document patient care and medication administration.
• Maintain a safe, clean, and comfortable environment for residents.
• Work collaboratively with nurses and other members of the healthcare team.

Requirements

• Active CNA certification.
• Valid QMAP certification.
• Previous experience in a long-term care or nursing home setting preferred.
• Strong communication and patient care skills.
• Ability to work 12-hour night shifts.
