Suncoast Hospice, a member of Empath Health, is seeking a caring and motivated CNA to provide personal care and tasks related to the activities of daily living for patients in their homes in the Seminole/Largo area, the office is located in Palm Harbor.

What youll Do
- The provision of hands-on personal care and assistance with activities of daily living.
- Assisting patients with taking medications that are ordinarily self-administered and have been pre-poured as designated in the Plan of Care.
- Hospice Aides must report changes in the patients medical, nursing, rehabilitative, and social needs to a registered nurse, as the changes relate to the plan of care. Hospice Aide must document the changes and to whom they were reported to.
- Following a patient's death collaborates with primary RN/Counselor and participates as needed in closure contact with family/caregivers.

What You'll Need
- Experience: One to three months related experience and/or training preferred.
- Valid Florida Board of Nursing certification (CNA License).
- Valid Florida State Driver's License
- For field-based positions, employees must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must provide proof of valid automobile insurance, a copy of which will be placed in the employee's HR file.
