Job Summary:

Manages a caseload of patients with psychiatric diagnoses and their families as part of an interdisciplinary team; contributes to the overall program by facilitating individual, group, and family meetings; leads groups; makes managed care calls; and establishes patient/discharge planning.

Essential Duties and Responsibilities

  • Supports and promotes the mission and values of Covenant Health Ministry.
  • Completes (biopsychosocial) assessment and treatment plan within 48 - 72 of admission, meets with the patient to review and get signature.
  • Meets with the patient within 48 hours of admission (weekend admits will be before the end of the day on Monday) and three times each week thereafter.
  • Meets with patients as part of an interdisciplinary team Mon-Fri for a minimum of 20 hours a week for documented face-to-face therapeutic contact.
  • Performs face-to-face evaluations as requested/needed, actively participates in treatment plans, and collaborates with community service providers as indicated.
  • Completes comprehensive discharge plan which includes, but is not limited to: treatment

recommendations, medication follow-up with a physician; appointments with outpatient care providers; and patient placement as indicated.

  • Communicates with the interdisciplinary team daily regarding patient progress or treatment-related issues.

  • Facilitates group meetings as scheduled, provides immediate crisis intervention services, and presents unit in-services as requested.

  • Intervenes when a patient is dangerous to self or others, utilizing least restrictive interventions, but may include seclusion and/or restraint under the direction of an RN.

  • Participates in program planning and development meetings, covers when a co-worker is out, and reports all patient concerns to the Charge Nurse or Unit Manager (PHP).

  • Completes clinical progress notes daily, documents treatment plan reviews within a week, significant events and managed care reviews as they occur, and discharge summaries within 24 hours of discharge.

  • Adheres to professional code of ethics, complies with staff conduct and confidentiality policies, maintains state and federal confidentiality regulations, and adheres to state guidelines of mandated reporting of child, adult, and/or elder abuse.

  • Obtains individual clinical supervision as needed, follows the chain of command and completes mandatory training and competencies within the stated timeframe.

  • Maintains licensure required for the position at all times.

  • Floats to other departments as needed.

  • Other duties may include:

  • Community notification of bed availability

  • Develop a working relationship with community partners

  • Monitor presentations for admission criteria and when appropriate, present them to the covering provider

  • Will assist with facility searches when unit is at capacity

  • Supporting student learning is a shared responsibility. Every team member is expected to contribute to creating a welcoming environment and to actively assist students in their educational experience, regardless of their specific role.

  • Other duties as consistent with this role.

Job Requirements

Job Knowledge and Skills

  • Strong written and verbal communication skills.
  • Strong interpersonal skills. Ability to handle high-stress situations.
  • Crisis Prevention Institute (CPI) training preferred or obtained before completion of orientation.
  • The ability to speak, read, write, and understand written instructions in English is required.

Education and Experience

  • Master's degree required.
  • Current Maine Social Worker/Clinician license required. LCSW, LMSW-CC, LCPC-C, LCPC, or LADC required.
