Qualifications

Education & Experience

  • A Masters' degree in Social Work, Psychology, or Counseling;
  • A Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPCC), or Licensed Marriage and Family Therapist (LMFT) credential or having registered as an Associate Social Worker (ACSW), Associate Professional Clinical Counselor (APCC) or Associate Marriage and Family Therapist (AMFT);
  • A Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPCC), or Licensed Marriage and Family Therapist (LMFT) credential and
  • At least 1 year of experience providing services to adolescents and their families

Essential Job Functions

Position Specific

  • Provides comprehensive, strength-based, culturally competent, flexible, effective, quality mental health services, including psycho therapeutic interventions, to eligible at-risk children, youth, and families.
  • Provides services to children and youth involved in the child welfare and Juvenile Justice systems meeting all County requirements as listed in program scope
  • Provides services to assigned children, youth, and families in the home, office, school, or other community based settings
  • Able to provide or arrange for safe transportation for youth and families to services as needed
  • Possesses specific cultural competence to work with and outreach to culturally diverse populations and communities
  • Ensures all clinical records are current and accurate, meeting organization and County timelines for submission
  • Provides and documents all Medi-Cal eligible services provided, including but not limited to assessment, plan development, individual therapy, family therapy, group therapy, collateral, rehabilitation, IHBS, ICC, case management, ICC-CFTs, and crisis intervention
  • Completes Intake, Core Assessment, Mid-Assessment, and Discharge packets for assigned client's meeting expected timelines
  • Accurately documents all billable and non-billable services provided, meeting expected timelines
  • Provides referrals and resources to children, adults and families, and acts as a liaison between child/family, assigned treatment team and external organizations
  • Utilizes Evidence Based Practices whenever possible, if clinically indicated
  • Ensures every assigned family has an individualized safety plan that maximizes the use of natural supports and community based resources
  • Attends all internal psychiatric appointments with assigned youth and caregivers as scheduled
  • Participates in the agency 24 hour family support on-call program
  • Participates in utilization, accreditation, peer review process as assigned
  • Responsible for meeting all outcomes as documented in Employee Development Plan
  • Works collaboratively with all internal programs, departments, and staff to provide support as needed
  • Maintains positive relationships with system partners, families and providers
  • Responsible for meeting all outcomes as documented in Employee Development Plan
  • Attends all staff meetings, clinical Supervision, and consultation meetings as scheduled
  • Participates in clinical supervision per BBS and County contract requirements
  • Coordinates with other services provided under the Medi-Cal model, acting in collaboration with other partners and ensuring documentation reflects standards that meet or surpass Stanford Youth Solutions + Sierra Forever families, state and federal Medi-Cal/Medicaid standard

  • Performs all duties in a manner consistent with the principles and values of the agency, while adhering to applicable professional codes of ethics, agency Policies and Procedures and County Mental Health Plan regulations
  • Model and communicate appropriate positive attitudes toward the agency's Mission, Vision, and Values
  • Work collaboratively with all agency programs and staff to provide support as needed
  • Utilize and maintain Microsoft Outlook calendar with all work-related details in order to manage time effectively and share calendar information with coworkers
  • Participate in on-going training to expand and develop professional skills
  • Perform other duties as necessary for the agency, as assigned

CompensationThis position may be filled at any level listed in this posting. Level determination is based on the role's minimum qualifications and the candidate's directly relevant experience and scope of responsibility. The posted salary bands represent the full pay range for each role and reflect opportunities for compensation growth over time. The typical hiring ranges reflect where we reasonably expect to make offers to new hires based on relevant experience, qualifications, and internal equity. Level I • Typical hiring range: $75,000 – $86,500 per year • Full salary band: $75,000 – $100,000 per year Level II • Typical hiring range: $80,000 – $92,000 per year • Full salary band: $80,000 – $105,000 per year Compensation offers are made through our established compensation framework to promote fairness, consistency, and internal equity.
