Description

JOB DEFINITION:The Per Diem Clinician provides crisis assessment, intervention, stabilization, and care coordination services to individuals presenting to the Community Crisis Center (CCC). As part of a multidisciplinary team, the clinician conducts comprehensive behavioral health assessments, safety planning and facilitates appropriate dispositions to the least restrictive level of care. The clinician supports the Community Crisis Center's mission of providing immediate, recovery-oriented behavioral health services while reducing unnecessary emergency department utilization and psychiatric hospitalization. EDUCATIONAL REQUIREMENTS:Master's degree in Social Work required or Mental Health Counseling. LMSW, LMCW, LMHC. LMFT, LMHC- D, LMFT, D EXPERIENCE REQUIRED:Minimum of one (1) year of experience providing behavioral health services in a crisis, emergency, inpatient, outpatient, mobile crisis, or community mental health setting preferred. Experience conducting behavioral health assessments, suicide risk assessments, safety planning, and crisis intervention strongly preferred.  DUTIES AND RESPONSIBILITIES:Gathers sufficient information for each client to make a diagnostic assessment necessary to determine assignment priorities and treatment plan or referral to the treatment resources in the community.Implements appropriate crisis services when needed to prevent decomposition and moves client toward an appropriate treatment plan and enhance adaptive functioning.Shares information about the Agency and its services to help clients and other interested parties participate in treatment decisions, set appropriate goals and better understand the functions of the center.Helps clients to become effective consumers of mental health and substance abuse services through information sharing, empowerment and transcendence of the stigma associated with those services.Establishes and maintains appropriate and timely clinical case records, including, but not limited to, treatment plans, progress notes and treatment plan reviews, documenting appropriate therapeutic intervention with each client to the Agency, funding, licensing and billing authorities.Work a minimum of 7 hours per month within the department; maximum 19 hours per week. Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progressComplete QI Training during onboarding and participate in QI refresher training as needed.Ability to cultivate a culture of inclusion for all employees that respects their individual strengths, views and experiences.
