The SHARE Clinician plays a critical role in ensuring the delivery of high-quality, recovery-oriented services. Working directly with individuals in community settings, the Clinician completes comprehensive assessments, evaluates behavioral health needs, and develops individualized treatment and rehabilitation plans, including the Individualized Rehabilitation Plan (IRP). The Clinician provides therapeutic support, crisis intervention, clinical consultation, and ongoing monitoring of participants' progress toward recovery goals.
In addition, the Clinician serves as the clinical leader for the SHARE team by providing guidance, oversight, and supervision to Case Managers. This includes supporting staff in identifying appropriate interventions, ensuring clinical best practices are followed, promoting compliance with documentation and regulatory requirements, and enhancing the overall quality of care provided to consumers. Through collaboration with internal and external providers, the Clinician helps ensure individuals receive coordinated, integrated services that promote housing retention, recovery, independence, and an improved quality of life.
