Sandra Eskenazi Mental Health Center, Indiana's first community mental health center, provides comprehensive care for emotional and behavioral problems, including severe mental illness and substance abuse. The Sandra Eskenazi Mental Health Center offers both inpatient and outpatient services, including several outreach centers as well as clinic- and community-based services.

The Focus of the Assertive Community Treatment Team

The Assertive Community Treatment team at the SEMHC is a recovery-oriented team, focused on helping the individuals in Marion county with the direst need. We provide essential service to individuals who have long struggled with serious mental illness, inadequate housing, chronic reincarceration, and long-term or frequent hospitalization. The team has been found effective in reducing negative outcomes among these groups, and helping them find recovery among the most difficult of circumstances. The team works together as a cohesive unit, providing whatever service is most needed in the moment, while also focusing on long-term recovery and personal goals. As a member of this team you might expect to spend your time out in the community seeing clients wherever it is safe, reasonable, and helpful for you to serve them, and coordinating with a large multi-disciplinary team of like-minded individuals oriented towards the service of others. The team includes psychotherapists, a team leader, a clinical supervisor, psychologist, peer recovery specialist, access coordinator, care coordinator, employment consultant, and psychiatric nurses, making it one of the best resourced teams within the CCBHC setting, with controlled shared caseload sizes of no more than 10 clients per staff. The Assertive Community Treatment Team (ACT) provides intensive wraparound services to adults with serious mental illness. Clients working with this team have commonly experienced recent hospitalization, incarceration, crisis services, housing instability, or a combination of these. This team is primarily community-based.

Where You Fit In

The Clinician performs a variety of individual, family and group therapeutic interventions utilizing evidence-based practices. This position manages a caseload of clients identifying therapeutic, rehabilitative and case management needs, engaging other team members in meeting these needs as indicated.

What To Expect

As a clinician on our ACT team, you'll spend most of your time in the community meeting clients where they are, including in their homes, residential facilities, boarding homes, and other community locations. You will support adults living with severe mental illness, with a focus on individuals who have recently been hospitalized or have complex needs that are best addressed through intensive community-based care.

Am I Qualified?

• Master's degree in Human Services field
• Candidates with a Master's of Social Work must have Licensed Social Work (LSW) and be working towards Licensed Clinical Social Work (LCSW)
• Candidates must have submitted a plan to obtain licensure at the soonest date possible within a maximum allowed of three years
• Valid Indiana driver's license that meets Eskenazi Health's driving policy requirements

Essential Functions and Responsibilities

• Embraces, understands, and operates under the Recovery Model, including AIDET (Acknowledge, Introduce, Duration, Explanation, Thank You) and the spirit of motivational interviewing (MI), and follows the levels of accountability
• Completes biopsychosocial evaluation (BPSE), integrating information gathered by the team and developing the integrated diagnostic summary; based upon the BPSE and in consultation with the treatment team, prepares Clinical Treatment plans, dates the treatment plan at intervals determined by guidelines, and consults with client, family, and treatment team throughout the process
• Manages a caseload of clients identifying therapeutic, rehabilitative and case management needs; engages other team members in meeting these needs as indicated
• Performs a variety of individual, family and group therapeutic interventions (as demonstrated competency allows) utilizing evidence-based practices
• Provides quality case management services as indicated by client need
• Monitors client progress and reflects this in the clinical documentation
• Evaluates and updates the treatment plan at intervals determined by guidelines, consulting with client family and team in this process
• Completes documentation of activities and written reports in an accurate and timely manner
• Identifies community resources and assists clients in their access to these resources
• Provides indirect service to external agencies and community through consultation and education
• Interprets and applies service and billing procedures according to client needs; completes billing according to guidelines for timeliness and accuracy
• Alters activities and behaviors to reflect and ensure adequate care which is appropriate to the age of the clients served
• Advocates for clients to ensure access to needed services which include developing community relationships and resources
• Meets agency standards of timely completion of all documentation
• Meets agency standards of client service hours
• Demonstrates commitment to professional growth by actively participating in clinical supervision

• Understanding of and ability to implement a strengths-based, community treatment model
• Strong interpersonal skills with ability to be both clear and concise in written and verbal communications
• Demonstrates solid knowledge of psychiatric/addiction diagnoses and understanding of their impact on behavior and all life domains
• Demonstrates solid knowledge of psychotropic medications and their uses and side effects
• Willingness and ability to work cooperatively in a multidisciplinary team
• Ability to use age-appropriate communication skills
• Ability to interpret and apply complex directives, information, and regulations
• Crisis intervention skills
• All individuals working for the Opioid Treatment Program must have experience and training to safely manage therapeutic services to patients and to provide therapeutic services to patients receiving Opioid treatment medication
