POSITION SUMMARY
The Clinical Team Coordinator (CTC) supports clinical staff, clinical charge nurses, leaders, and pertinent office staff by maintaining accurate and timely records for patient referrals, admissions, and denials. The CTC is committed to continuous quality improvement by entering timely and accurate data for reporting and measuring of outcomes and goals, and collaborating across departments, work functions and procedures to ensure prompt and appropriate service for all internal and external customers.
PRIMARY ACCOUNTABILITIES
- Works independently throughout the system to access and update information; communicates accurate clinical information, provides appropriate data, and collaborates within and beyond the team to support the delivery of quality services.
- Prioritizes patients appropriately to ensure those with greatest needs are seen in the correct order and communicates completion of work to CTC leader to support and ensure compliance with departmental standards.
- Responds appropriately to unexpected, stressful situations and professionally interact with customers and associates.
- Identifies solutions to complex problems, sets priorities and manages time efficiently, seeking advice and guidance from leaders as needed.
- Suggest ways to improve/enhance workflow, procedures, customer delight, and quality outcomes.
Work Experience
MINIMUM QUALIFICATIONS
- Education: High School Diploma or equivalent.
- Work Experience: One (1) year experience in Customer Support role.
- Licensure: None
- Certification: None
- Background Check: If hired into Hospice: Satisfy the State of Florida, Agency for Healthcare Administration (ACHA) Level II Background screening.
- Skills/Knowledge/Abilities:
- Basic computing skills with proficiency in MS Office - Outlook, Word,
- Excel, PowerPoint.
- Excellent auditory, verbal and written communication skills.
- Strong critical thinking skills to identify solutions and prioritize multiple demands.
PREFERRED QUALIFICATIONS
- Education: Associate's degree in relevant field.
- Work Experience: One (1) year Customer Support experience in Healthcare setting.
- Skills/Knowledge/Abilities:
- Familiarity with regulatory documentation.
- Knowledge of medical terminology, coding (ICD-10, HCPCS, CPT) and accounting principles.
PHYSICAL REQUIREMENTS
- Majority of time involves sitting or standing; occasional walking, bending, and stooping.
- Long periods of computer time or at workstation.
- Light work that may include lifting or moving objects up to 20 pounds with or without assistance.
- May be exposed to inside environments with varied temperatures, air quality, lighting and/or low to moderate noise.
- Communicating with others to exchange information.
- Visual acuity and hand-eye coordination to perform tasks.
- Workspace may vary from open to confined.
- May require travel to various facilities within and beyond county perimeter; may require use of personal vehicle.
