Job Description

Job Summary:

The Supervisor of Clinical Services is responsible for having primary authority and responsibility for maintaining the quality of clinical services. The agency employs one full-time supervisor of clinical services for each fifteen (15), or less, full-time or full-time equivalent professional direct service staff.

The supervisor of clinical services may also serve as the administrator in agencies with six (6) or less full-time or full-time equivalent professional direct service staff.

Any absence of the supervisor of clinical services for longer than one month must be reported to the commissioner. A registered nurse who has at least two (2) years' experience in a home health care agency, shall be designated, in writing, to act during any absence of the supervisor of clinical services whenever patient care personnel are serving patients.

Qualifications:

The qualifications for the Supervisor of Clinical Services are as follows:

A registered nurse with an active license to practice nursing in this state, and one of the following:

1. A master's degree from a program approved by the National League for Nursing or the American Public Health Association with a minimum of one year (1) full-time clinical experience in a home health agency or related community health program which included care of the sick at home; or
2. A baccalaureate degree in nursing and a minimum of three (3) years of full-time clinical experience in nursing, at least (1) one of which was in a home health agency or community health program which included care of the sick at home; or
3. A registered nurse who has been continuously employed in the position of supervisor of clinical services in a home health agency in this state since January 1, 1979; or
4. A diploma in nursing or an associate's degree in nursing and

1. A minimum of three years of full-time or full-time equivalent clinical experience in nursing within the past five years, at least one year of which was in a home health care agency or community health program which included care of the sick at home; and
2. Evidence of certification by the American Nurses' Association as a community health nurse or completion of at least six credits received within two years in community health nursing theory or six credits in health care management from an accredited college or university program or school of nursing.

Primary Responsibilities:

The supervisor's responsibilities include, but are not limited to:

1. Coordination and management of all services rendered to patients and families by direct service staff under his/her supervision;
2. Supervision of assigned nursing personnel in the delivery of nursing services to patients and families;
3. Direct evaluation of the clinical competence of assigned nursing personnel and participation with appropriate supervisory staff in the evaluation of other direct service staff;
4. Participation in or development of all agency objectives, standards of care, policies and procedures affecting clinical services;
5. Participation in direct services staff recruitment, selection, orientation and inservice education;
6. Participation in program planning, budgeting and evaluation activities related to the clinical services of the agency.

The following description of job responsibilities and standards are intended to reflect the major responsibilities and duties of the job but is not intended to describe minor duties or other responsibilities that may be assigned from time to time.

1. Assumes primary responsibility for clinical services and personnel.

1.1 Oversees and supervises daily clinical staff in providing home health care to patients.

1.2 Ensures adequate clinical staffing.

a. Arranges nursing schedule 7 days/week including weekends and holidays.

b. Ensures caseload distribution among nurses as appropriate.

c. Maintains the productivity standards set forth by agency.

d. Arranges for per diem staff as needed.

1.3 Manages and supervises personnel.

a. Interprets and ensures clinical staff compliance with all agency policies.

2. Screens and interviews new employees.
3. Provides supervisory on-call per agency directives.

1.4 Monitors documentation of clinical services.

1. Reviews all clinical documentation prepared by clinical staff in delivering service to patients.
2. Reviews patient records from admission through discharge to determine accuracy, compliance with documentation standards and appropriateness and duration/frequency of clinical service.

1.5 Ensures competency of clinical staff.

a. Participates in the selection of qualified applicants.

b. Provides orientation to new employees.

c. Conducts competency testing according to agency policies.

3. Conducts performance evaluations per agency timeframes.
4. Plans and conducts meetings with clinical staff on a regular basis.

g. Conducts on-site home visits with clinical staff as part of routine supervision and evaluation of staff performance.

2. Ensures compliance with all Federal and State regulations and standards for clinical

service and staff.

2.1 Demonstrates knowledge of current home health rules and regulations pertaining to clinical
practice.

2.2 Demonstrates knowledge of current rules and regulations pertaining to home health reimbursement.

3. Participates in continuing education of clinical staff.

3.1 Acts as a resource person in the field of clinical and/or psychiatric nursing to the agency.

3.2 Participates in the interpretation and education of staff about agency policies, procedures,
objectives and standards of care.

3.3 Participates in the agency's staff development program.

4. Participates in self-evaluation and professional development.

4.1 Identifies strengths and areas to be developed.

4.2 Sets goals for personal and professional growth.

3. Attends appropriate in-services as warranted.

4. Remains current in-home health regulations and industry issues.
