As a valued member of our multidisciplinary care team, you'll work alongside physicians, advanced practice providers, nurses, and support staff to address the emotional, psychological, social, and practical needs of patients throughout treatment, survivorship, and end-of-life care. Clinical Social Worker (LCSW)

Full-Time | Monday – Friday | Onsite | Winchester, VA

$83,000 – 96,000 per year. The actual hiring rate may vary based on several factors, including your previous work experience, educational background, the responsibilities of the position, and demonstrated performance. Responsibilities:

• Provide therapeutic counseling, crisis intervention, and emotional support to patients and families.
• Conduct psychosocial assessments and develop individualized support plans.
• Participate in distress screening follow-up, survivorship care, and advance care planning discussions.
• Connect patients with financial, community, and supportive resources.
• Serve as a patient advocate and help navigate healthcare and community systems.
• Facilitate support groups and collaborate with community organizations.
• Partner with providers and clinical staff to address psychosocial barriers to care.
• Maintain timely and accurate documentation in the electronic medical record.

Qualifications:

• Master's degree in Social Work (MSW) from a CSWE-accredited program. 
• Current Virginia Licensed Clinical Social Worker (LCSW) license in good standing.
• Minimum of three (3) years of clinical social work experience in a healthcare setting.
• Experience in oncology, palliative care, hospice, behavioral health, or other medically complex patient populations preferred. 
• Experience facilitating support groups, patient education programs, or community-based support services.
