Position Summary/Department Summary:This 32-hour-per-week social work position providing a broad range of clinical social work services within the innovative STRiDE Program in the Boston Children’s Hospital Type 1 Diabetes Program. The STRiDE (Support, Tools, Resources in Diabetes Empowerment) Program provides an additional layer of individualized support to youth with Type 1 Diabetes and their families, helping them identify and overcome psychosocial, socioeconomic, and healthcare-related barriers that may interfere with successful diabetes management and engagement in care. The STRiDE team is a multidisciplinary group currently consisting of two pediatric endocrinologists, two diabetes nurse educators, two diabetes resource nurses, a quality improvement/analytics expert, and an administrative assistant. We are seeking to expand the team and further strengthen our ability to support children, adolescents, and families through the addition of a dedicated clinical social worker.
Key Responsibilities- Conduct comprehensive psychosocial assessments and provide clinical interventions for children, adolescents, and families living with new or existing diagnoses of Type 1 Diabetes.
- Implement clinically sensitive and evidence-informed clinical interventions to promote family engagement in care to and to improve health and psychosocial outcomes.
- Facilitate access to appropriate community resources and support services while collaborating with the broader diabetes care team to address patients’ and families’ psychosocial needs.
- Participate in STRiDE leadership meetings to support program evaluation, quality improvement initiatives, and ongoing program development.
- Participate in STRiDE clinical case conferences to collaborate with participants’ multidisciplinary care teams and develop comprehensive, coordinated plans of care.
- Collaborate closely with the Endocrine Mental Health Team to provide endocrine-related behavioral health support, consultation, and clinical coverage.
Minimum QualificationsEducation: Master’s degree in social work
Experience:- Social work experience in medical environment and with child protection experience preferred.
- Flexibility and ability to work in fast-paced, dynamic environment.
- Excellent teamwork and communication skills necessary.
- Spanish-speaking preferred but not required.
Licensure/Certification:- LCSW Social Work licensure in the state of Massachusetts, LICSW preferred
