The Integrated Care Center Clinic is a federally qualified health centers that provide primary care, behavioral health, addiction treatment, psychiatry, dental care, and more. We provide care to those most in need including those who are suffering from homelessness, mental illness, substance use, mental illness, and chronic medical conditions. We provide team based care in an integrated care setting. We are looking for a mission driven, service leader who works well in a team, has experience with managing and supporting staff, champions quality improvement, leads change, and is excited to innovate.

The Clinical Social Worker provides behavioral health therapy and integrated case management to individuals experiencing unsheltered homelessness and complex medical, psychiatric, and social needs. This role combines field-based case management with direct clinical mental health services and plays a key role in strengthening and expanding the Mobile & Street Medicine program's behavioral health capacity. This position is highly collaborative, flexible, and hands-on. The ideal candidate thrives in dynamic field environments, enjoys wearing many hats, and brings creativity, leadership potential, and a commitment to building new systems of care alongside a multidisciplinary team. This is a full-time, in-person, field-based position. This position may be filled by an ASW practicing under clinical supervision, which will be provided by an LCSW employed by HR360, or by an independently licensed LCSW.

QUALIFICATIONS:
Education, Credentials, and Experience
Desired
Enhanced Care Management (ECM) experience, or other intensive case management equivalent.
Experience in inpatient or hospital-based social work.
Leadership experience or demonstrated leadership potential.
Experience in homeless healthcare and Street Medicine strongly preferred.
Experience working with co-occurring mental health and substance use disorders.
Must complete background check and Livescan.
