The Clinical Social Worker 3 (CSW 3) primary responsibilities are psychosocial assessment, crisis intervention, counseling, brief therapeutic interventions, consultation, education, community linkages, supportive services, and to facilitate the plan of care through the health care continuum for patients, families, and staff, with particular focus on high-risk populations. The CSW 3 acts as a consultant to the clinical team, service lines and other departments and participates in program development and quality improvement initiatives. The CSW 3 will conduct legal and ethical billing for services when applicable and appropriate. The CSW 3 will primarily practice at the McClymond's High School FQHC Health Center location and at the Claremont Primary Care Clinic but also be available to the Castlemont High School FQHC Health Clinic, Claremont TEEN Adolescent FQHC site, FQHC Mobile Clinic as needed.
The CSW 3 will support students and community members in and around McClymonds High School as well as patients in the Encore Medical Clinic, a specialty clinic within Claremont Primary Care, which services unstably housed children and their families. The CSW 3 will practice within the scope of UCSF's mission statement: Caring, Healing, Teaching, Discovering; and the values statement of UCSF's Everyday PRIDE: Professionalism, Respect, Integrity, Diversity, Excellence.
The CSW 3 will participate in department education and practice improvement activities. DUTIES & ESSENTIAL JOB FUNCTIONS
Provide psychosocial assessment, crisis intervention, counseling, information, referral and linkage to community referrals for patients, their families and staff.
Active member of the interdisciplinary team, specifically on complex psychosocial cases, contributing to team goal setting and plan of care.
Inpatient CSW 3 develops and implements a safe discharge plan while the outpatient CSW 3 facilitates continued care planning. Focus for both is to address the biopsychosocial needs of the patient and their family while in collaboration with the interdisciplinary team members.
Assess, intervene with and report domestic violence, child/elder abuse and other reportable situations in accordance with legal mandates and UCSF policy and procedures.
Screen for suicidal/homicidal ideation and follow legal mandates and UCSF policy and procedures to assure patient safety. Collaborate with UCSF Psychiatry, the BERT team and other appropriate teams on cases meeting criteria for psychiatric holds or complex behavioral health concerns.
Consult on challenging custody issues to clarify decision-making capacity, identify surrogate decision maker(s), facilitate completion of Advance Health Care Directive and/or POLST; when appropriate coordinate application for probate conservatorship or court order.
Knowledge and utilization of the Diagnostic and Statistical Manual (DSM).
Knowledge and skill with substance use and dependence assessment and intervention strategies, mental illness, end of life/bereavement, benefits and entitlements, and adjustment to illness.
Complies with Department standards for timely submission, accuracy and completeness of documentation in the Medical Record and collects all required statistical data.
