Starting Pay: 85,000 to 90,000 (Salaried role and pay is based on previous experience)
5 days per week schedule
On-call manager rotation
Position Purpose:
The Clinical Rehab Nurse Manager is an RN Nurse who serves in the capacity of a
coordinator with assessing, planning, implementation, monitoring and evaluation
of the options and services required to meet the residents’ health service
needs.
Primary Job Duties:
- Clinical Rehab Nurse Manager Participates in welcoming new residents and their
families to the community.
- RN Initiates and supervises the completion of Physician
Certification & recertification for rehab stay.
- Manages resident and family concerns in a calm &
professional manner.
- Serves as a nursing representative for PAN, Bundle
& Medicare meetings.
- Participates in resident baseline and comprehensive
care plan for continuity of care.
- Preparation, review and submission of insurance
documentation, authorization, appeals and audits.
- Assessing, implementing, and evaluating resident
individual level of care needs and ensuring that is reflected in resident care
plan.
- Monitors daily Nursing documentation to ensure
resident condition and treatment is being appropriately documented.
- Participates in the Nurse on Call rotation.
- Other duties as assigned.
Requirements:
- Registered Nurse RN license in good standing
- 3+ years clinical experience
- Minimum 1 year experience in long term care.
