    Position Overview

    First Choice Primary Care, Inc., a Federally Qualified Health Center (FQHC), is seeking a Clinical Quality Technician to support clinical quality initiatives across the organization. The Clinical Quality Technician assists with the collection, monitoring, tracking, and reporting of clinical quality data and measures. This position works closely with the Director of Quality and clinical teams to support quality initiatives, identify opportunities for improvement, and promote compliance with established clinical quality standards.

    The Clinical Quality Technician also assists with Patient-Centered Medical Home (PCMH) program activities and works with the Director of Quality to support First Choice Primary Care's efforts toward and maintenance of the National Committee for Quality Assurance (NCQA) recognition/accreditation. This position plays an important role in supporting clinical quality reporting, staff education and training, documentation, monitoring, and continuous quality improvement activities.

    This is an 8am - 5pm on-site job that involves traveling to other locations as needed.

    Essential Duties and Responsibilities

    • Assists Director of Quality to foster compliance with standards and regulations in the areas of Quality Assurance, Uniform Data System (UDS) data analysis, and PCMH benchmarks.
    • Identifies barriers to implementing PCMH model and works with staff to develop solutions to address barriers.
    • Performs point of care blood glucose testing and documents results in patient's electronic medical record.
    • Collects blood specimens for Hemoglobin A1c testing to support diabetes management and quality initiatives
    • Conducts Healthcare Effectiveness Data and Information Set (HEDIS) chart audits and validates documentation for accuracy and completeness.
    • Reviews ICD-10 and CPT coding to support risk adjustment and quality reporting efforts.
    • Participates in interdisciplinary meetings with population health, utilization management, and provider engagement teams.
    • Conducts patient outreach to close preventive and chronic care gaps for Medicare and Medicaid populations.
    • Educates patients on preventive screenings and chronic disease management recommendations.
    • Utilizes payer portals for care gap closure and suspect condition validation.
    • Assists with reporting, monitoring, and tracking performance metrics across multiple quality programs.
    • Other duties as assigned by Executive Leadership or the Director of Quality.

    License/Certification(s)

    • Certified Medical Assistant required.

    Qualifications

    • High school diploma or equivalent required; associate degree or higher in healthcare, health information management, public health, quality improvement, or a related field preferred.
    • Previous experience in a healthcare or clinical environment preferred.
    • Experience with clinical quality measures or quality improvement programs.
    • Knowledge of Patient-Centered Medical Home (PCMH), National Committee for Quality Assurance (NCQA), Healthcare Effectiveness Data and Information Set (HEDIS), Uniform Data System (UDS), or other healthcare quality programs.
    • Experience working with eClinicalWorks (eCW) or an electronic health record (EHR) system.
    • Strong attention to detail and accuracy.
    • Ability to collect, organize, track, and report data.
    • Strong organization and time-management skills.
    • Proficiency with Microsoft Office, particularly Excel.
    • Effective written and verbal communication skills.
    • Ability to work independently and collaboratively with clinical and administrative teams.
    • Ability to maintain confidentiality and handle sensitive information appropriately.

    Physical Demands and Working Conditions

    Frequent mobility and/or sitting required for extended periods of time. Prolonged computer and data entry requirements. Requires manual dexterity to operate keyboard and other office equipment. Requires corrected vision and hearing to normal range. High stress position due to work deadlines. Requires some overtime and occasional weekend commitment as the need may arise based on deadlines for program reporting requirements.
