Job Summary
At North Central Nursing Clinics (NCNC), a federally qualified health center (FQHC), youll have the opportunity to make a meaningful difference by expanding access to high-quality healthcare and improving the well-being of the communities we serve.
Reporting directly to the Medical Director, the Clinical Quality & Performance Lead / Nurse Practitioner provides primary healthcare services to NCNC patients and serves as a clinical leader for organizational quality improvement, population health, and value-based care initiatives.
This hybrid position dedicates approximately 50% of its responsibilities to direct patient care and 50% to clinical quality and performance improvement. In the clinical quality role, the position collaborates with the Medical Director and the Director of Quality to translate organizational quality priorities, clinical performance data, regulatory requirements, and value-based care objectives into effective clinical practice.
