Job Requirements
This position will be supporting the Emergency Department at Holmes Regional Medical Center. The schedule will vary and will include 2nd shifts.
POSITION SUMMARY
The Clinical Pharmacy Specialist acts on a facility level to assist the system clinical pharmacy manager in planning, developing and coordinating clinical pharmacy services and ensuring that the pharmacy provides optimal clinical services on a daily basis at their institution. The Clinical Pharmacy Specialist facilitates the development, implementation, and maintenance of systems that promote desirable patient outcomes through the use of safe, cost-effective drug therapies.
PRIMARY ACCOUNTABILITIES
Develops and presents comprehensive drug reviews, medication use evaluations, sensitivity and adverse reaction reports, and pharmacy interventions to key committees, supporting safe and effective medication management.
Reviews and monitors targeted medication use to ensure compliance with pharmacoeconomic guidelines and participates in the creation and implementation of evidence-based treatment protocols and formulary updates.
Discloses, and current best practices and relevant clinical information from external and internal sources across the organization.
Monitors medication regimens for efficacy, safety (including contraindications and interactions), and appropriateness, intervening as necessary to optimize patient outcomes.
Leads and participates in quality improvement initiatives both within the pharmacy and hospital-wide, documenting clinical activities and outcomes to drive continuous improvement.
Evaluates and advances clinical pharmacy programs and systems to maximize efficiency, effectiveness, and patient care, including the development and direction of clinical pharmacy services.
Provides timely drug information and education to healthcare professionals and assists with staff training and performance evaluation.
Coordinates pharmacy involvement in community education programs and fosters collaborative relationships within the department and with external partners.
Identifies and addresses cost-effective medication use opportunities in collaboration with medical staff, offering therapeutic alternatives and supporting formulary and policy development.
Analyses and investigates clinical pharmacy activities, including documentation and financial impact, and prepares reports as needed to inform decision-making and support organizational goals.
Maintains current pharmacist licensure and professional competence through ongoing education, and ensures patient and departmental confidentiality at all times.
Partners effectively with all levels of Associates, and contributes to the advancement of pharmacy practice through organizational and professional involvement.
