Aligned with target interdisciplinary programs, the Clinical Pharmacist Specialist provides medication therapy management within patient care services, ensures pharmaceutical care programs are appropriately integrated across the hospital's clinics, and serves as a departmental resource and liaison to other departments while providing education and training to health care providers. Complete critical patient monitoring and chart review to detect, prevent, and mitigate drug-related problems (e.g., improper drug or dose selection, adverse reactions, drug interactions, treatment failures); identify high-risk patients and intervene to improve adherence, outcomes, and safety.
