Completes critical patient monitoring and reviews patient profile/ chart to identify, prevent, or mitigate drug- related problems, Partners as an active member of multidisciplinary team and collaborates with healthcare providers within decentralized patient care areas to provide patient- centered care. 2 years For Infectious Diseases/Antimicrobial Stewardship roles: ASHP - accredited PGY2 Infectious Disease residency OR minimum of 2 years of equivalent clinical pharmacy experience in infectious diseases/antimicrobial stewardship, required.

What We're Looking For:

• Education:
  • 4 Year / Bachelors Degree, required. Bachelor's Degree in Pharmacy, Foreign Pharmacy Graduate Equivalency Committee (FPGEC) Certification.
  • PharmD, preferred. 
• Experience: 
  • Postgraduate year 1 residency or fellowship OR two years of experience as a registered pharmacist in health system pharmacy. Oncology experience preferred for oncology roles, required.
  • 2 years For Infectious Diseases/Antimicrobial Stewardship roles: ASHP - accredited PGY2 Infectious Disease residency OR minimum of 2 years of equivalent clinical pharmacy experience in infectious diseases/antimicrobial stewardship, required. 
• Licensure/Certification:
  • Current Pharmacy licensure in appropriate state(s) or must be completed within 30 days of start date by either examination or reciprocity, required.
  • Board of Pharmacy Specialties Certification or other approved certification must be obtained and continuously maintained within 2 years of start date as applicable to the role
• Additional Skills (required):
  • Other duties as outlined in Novant Health and facility specific policies and procedures. Ability to drive/travel to multiple facilities/locations as needed. Ability to work as part of a high performing, change ready and resilient team.

What You'll Do:

Essential functions include:

• Flexibility in working in acute care capacities with adults and pediatrics in the emergency department
• Facilitates pharmaceutical care services, medication utilization systems within assigned services and care areas to ensure drug utilization activities are aligned with patient care needs, evidence- based best practices and regulatory standards
• Completes critical patient monitoring and reviews patient profile/ chart to identify, prevent, or mitigate drug- related problems,
• Partners as an active member of multidisciplinary team and collaborates with healthcare providers within decentralized patient care areas to provide patient- centered care
• Makes appropriate evidence-based, patient- centered medication recommendations
• Participates in the management of medical emergencies for adult and pediatric patients
• Provides discharge medication review, reconciliation, and counseling as appropriate
• Maintains proficiency in and actively engages in central dispensing functions and operational programs
• Oversees pharmacy operations and technician practice/activities
• Maintains proficiency in preceptor roles and actively emulates education/ mentorship skills
