Position Title: Pharmacist 3

Location: Remote
Duration: 3 Months

Payrate: $60 - 65/hr on W2
Schedule: Monday Friday, 8:00 AM 6:00 PM Central Time; shifts will fall within these hours. Occasional weekend work may be required.

Position Summary

We are seeking an experienced Pharmacist to support remote pharmacy operations focused on home infusion, IVIG, and hemophilia services. This role will be responsible for prescription review and clarification, clinical assessment, patient counseling, provider communication, and ensuring safe and appropriate medication therapy.

The ideal candidate will be comfortable working in a specialized pharmacy environment, managing changing priorities, communicating with patients and healthcare providers, and working independently while collaborating closely with a small, supportive team.

Key Responsibilities

• Review and clarify prescriptions received from provider offices for home infusion services.
• Verify prescriptions contain all necessary information, including medications, pre-infusion medications, flushes, infusion duration, nursing requirements, supplies, and pump requirements.
• Evaluate medication orders for appropriate dosage, drug selection, potential interactions, route of administration, and stability.
• Provide individualized clinical support for patients with complex and rare disease conditions.
• Provide patient counseling, education, and care plans.
• Support physicians, nurses, providers, and home infusion teams with clinical and technical pharmacy guidance.
• Conduct inbound and outbound calls with patients, providers, physicians, and nursing staff.
• Assess patient response to therapy, compliance, medication regimens, and potential adverse events.
• Identify and provide appropriate clinical interventions.
• Document communications and clinical activities accurately in patient records.
• Ensure patient information, including allergies, medical history, medications, and relevant laboratory results, is appropriately reviewed and maintained.
• Support medication dispensing, receiving, compounding, and related pharmacy activities as applicable.
• Monitor medication quantities and inventory to help prevent waste.
• Participate in quality improvement and performance improvement initiatives.
• Assist with training and supporting pharmacy technicians, interns, and other pharmacy personnel.
• Maintain compliance with applicable pharmacy policies, procedures, and regulatory requirements.
• Complete required education, training, competency, and skills assessments.
• Participate in on-call responsibilities as required.
• Perform other duties as assigned by pharmacy leadership.

Required Qualifications

• Bachelor s degree in Pharmacy with 3+ years of mail-order, retail, or hospital pharmacy experience OR PharmD.
• Active Pharmacist License in Texas, Missouri, or Florida.
• 3+ years of professional pharmacy experience.
• Strong computer proficiency.
• Ability to analyze prescription and patient-related information accurately.
• Strong customer service and communication skills.
• Ability to work independently while collaborating effectively with a team.
• Ability to adapt to changing workflows and prioritize multiple tasks.
• Strong attention to detail and commitment to patient safety.

Preferred Qualification

• IGNS certification preferred
