    Position Summary: -The Clinical Nurse Navigator I is a registered nurse responsible for the seamless coordination of care across the continuum for patients with complex, integration sensitive diagnoses. -The Navigator is a patient advocate who coordinates care in conjunction with the physicians from time of diagnosis or admission through discharge or up to end of life. -Educates patient and family/signi cant others on the health care experience; implements, coordinates, monitors and evaluates care options, services and resources. -Links family/patient to appropriate resources as needed. -Proactive in nding ways to enhance the patient experience clinically and in service. -Provides an exceptional patientexperience. -The Nurse Navigator assists in quality improvement initiatives by gathering, tracking, and researching quality metrics. -Monitors outcomes, e.g. patient satisfaction, compliance with treatment regimen and loyalty. -Interacts with department Nurses, may orient and train incoming Clinical Nurses and Patient Care Assistants. -Performs other duties as assigned.

    Skills
    ====
    Required Skills & Experience:
    -Five (5) years in nursing and 3 in area of specialty.
    -Minimum of Five (5) years experience in nursing and Three (3) years of experience in area of specialty.
    -Neurosciences: Fulfills mandatory stroke education requirements per certification agency.
    -Oncology: One plus (1+) years of Oncology nursing experience
    Preferred Skills & Experience:
    -Acute Rehab: Prior Navigator experience.
    -Care Management: Prior Sepsis RN experience preferred. Prior experience as a Case Manager, Navigator, or Health Educator.
    -*** Orthopedic Institute (HOI): Prior Orthopedic Navigator Experience.
    -Community Health: Bilingual skills, especially Spanish are.
