Clinical Nurse Liaison (RN) – Hospice & Community Outreach - Full -Time

What You’ll Do<\/span>

Key Responsibilities<\/span>
<\/p>

• Build and maintain relationships with physicians, hospitals, and community partners to grow hospice services<\/span>
  <\/li>
• Serve as the primary RN for General Inpatient (GIP) hospital admissions and routine hospice admissions<\/span>
  <\/li>
• Evaluate patients for hospice eligibility in collaboration with physicians and the interdisciplinary team<\/span>
  <\/li>
• Develop and execute community outreach initiatives while tracking activity in Salesforce<\/span>
  <\/li>
• Collaborate with Director of Business Development, Chief Nursing Officer , and the IDT to support quality patient care<\/span>
  <\/li>
• Ensure compliance with all local, state, and federal regulations, including Texas BON standards<\/span>
  <\/li>
• Promote the organization's mission, values, and philosophy<\/span>
  <\/li>
• Perform other duties as assigned<\/span>

  What We Are Looking For<\/span>

  Qualifications<\/span>
  <\/p>

  • Graduate of an accredited Diploma, Associate, or Bachelor's School of Nursing<\/span>
    <\/span><\/li>
  • Current Texas Registered Nurse (RN) license; Current CPR certification<\/span>
    <\/span><\/li>
  • Minimum of 2 years of RN experience in a clinical setting required<\/span>
    <\/span><\/li>
  • Hospice or home health experience preferred<\/span>
    <\/span><\/li><\/ul>

    <\/p>

    • Strong clinical assessment, nursing, communication and relationship -building abilities<\/span>
      <\/span><\/li>
    • Experience with timely, compliant documentation & Proficiency with Salesforce/CRM preferred<\/span>
      <\/span><\/li>
    • Ability to work independently, collaboratively, and manage priorities<\/span>
      <\/span><\/li>
    • Emotional resilience to support patients and families during hospice transitions<\/span>
      <\/span><\/li>
    • Current TB testing, Hepatitis documentation & Ability to pass a criminal background check<\/span>
      <\/span><\/li>
    • Valid Texas driver's license, reliable transportation and current auto liability insurance<\/span>
      <\/span><\/li><\/ul>
