Position Summary: EXCITING NEW OPPORTUNITY IN SPECIALTY PHARMACY

As a Clinical Nurse Liaison with Maxor National Pharmacy Services, you will perform collaborative duties to assess, plan and coordinate continuum of care for select members with complex medical or behavioral health conditions or are at high risk in order to promote quality, cost effective care.   

Job Duties: 

  • Assist as a liaison with patients and their families to physicians, staff, and other health care professionals to coordinate patient care across all settings and ensure high quality outcomes.
  • Advanced review of admissions through on-site and telephonic review to ensure medical necessity and appropriate level of care.
  • Develop, assess and adjust as necessary the care plan and promote desired outcome.
  • Coordinate services between Primary Care Physician (PCP), specialist, and other medical and non-medical providers as necessary to meet the complete medical socio-economic needs of clients.

Required Qualifications:

  • Current NV state RN license.  CCM (certified Case Manager) or other clinical certification preferred.
  • Minimum two years Home Health experience or experience with patients that have rare and/or chronic diseases.
  • Excellent written and verbal customer service skills to interact with key stakeholders and patients.
  • Have experience with Microsoft Office, Excel, Outlook and Word to document and track patient care activity.
