    Responsibilities:
    • The Clinical Navigator RN coordinates and facilitates patient care across the continuum of care for designated patient populations.
    • The position serves as a clinical resource, patient advocate, and liaison among patients, families, providers, and interdisciplinary teams to promote high-quality outcomes and a positive patient experience.
    • This role supports care coordination, patient education, clinical program initiatives, and evidence-based practice standards.
    • The Clinical Navigator RN contributes to improved clinical outcomes through effective communication, collaboration, and patient-centered care management. 
    Qualifications:

    Job Requirements:

    • Graduate of an accredited school of nursing. 
    • Current Registered Nurse (RN) license. 
    • BLS certification must be obtained within 14 days of hire or transfer into the role and prior to providing direct patient care

    Preferred Qualifications

    • Bachelor of Science in Nursing (BSN). 
    • Experience in cardiology, vascular services, critical care, cardiac surgery, or emergency services. 
    • Public speaking or community education experience. 
    • Knowledge of quality improvement methodologies. 
    • Skill in program coordination and patient navigation activities.
