We are seeking a full-time Clinical Manager for the Columbus area who can help improve the well being of our patients and their families and has the heart and skills to work with home bound patients.

• Work with the physician for medication needs and changes to the plan of care.
• Manage other disciplines such LPNs, aides, therapy staff to give the best care needed for each patient.
• Ability to complete OASIS, recert, d/c, ROCs documentations within an EMR system
• Help decrease the need for re-hospitalization by being actively involved in the patient's plan of care to assess any changes that may be needed.
• When appropriate teach and counsel family members about care and processes for disease/injuries.
• Attend orientation, meetings and CEUs,
