Anuco Home Health Care LLC is looking for a dedicated and experienced Clinical Manager to join our growing team in Chicago, IL. Responsibilities:

• Oversee and coordinate daily clinical operations to ensure high-quality patient care and regulatory compliance
• Supervise, mentor, and evaluate clinical staff including nurses, therapists, and home health aides
• Conduct patient assessments, care plan development, and ongoing case management
• Ensure adherence to state, federal, and accreditation standards for home health care
• Collaborate with physicians, interdisciplinary teams, and families to optimize patient outcomes
• Monitor clinical documentation for accuracy, completeness, and compliance
• Identify and implement quality improvement initiatives to enhance care delivery

Requirements:

• Active Registered Nurse (RN) license in the state of Illinois required
• Minimum 2–3 years of clinical experience in home health, community health, or a related setting
• Previous supervisory or management experience strongly preferred
• Strong knowledge of Medicare/Medicaid home health regulations and OASIS documentation
• Excellent leadership, communication, and organizational skills
• Proficiency with electronic health record (EHR) systems
• Compassionate, patient-focused approach with a commitment to quality care
